In the Southwest Health District, we offer Perinatal Case Management (PCM) services exclusively for pregnant women. PCM is aimed at supporting, enhancing, and coordinating essential health services for both mothers and babies.
Our main goal with PCM is to decrease infant illness and mortality rates by improving pregnancy outcomes. Here’s how our PCM services in the Southwest Health District are personalized:
- We conduct individualized assessments, develop plans, link individuals to necessary services, and ensure follow-up.
- PCM starts early in pregnancy to establish support networks for required resources.
- Our comprehensive care covers economic, nutritional, psychosocial, educational, and medical/health aspects throughout pregnancy and the postpartum period.
- We provide comprehensive preventive services to pregnant women and their infants.
- We make referrals for medical costs, prenatal care, prenatal education, WIC services, and other essential services.
Presumptive Eligibility (PE) for Pregnant Women
The Presumptive Eligibility for Pregnant Women program is designed to provide medical assistance coverage during the processing period and to remove barriers to the availability of prenatal care that is critical in positive affect the birth outcome and health of mothers. As soon as you find out you are pregnant, you can apply to see if you are eligible for PE Medicaid to help cover your costs throughout your pregnancy. Costs that are covered can include provider visits, prescription medicines, labor and delivery, and inpatient and outpatient hospital services.
Eligibility Requirements
In order to be eligible to receive PE Medicaid, an applicant must meet the following criteria:
- You must be pregnant.
- You must meet income requirements (listed here).
- You must be a U.S. citizen or meet certain immigration status requirements
